Zentar Health & Social Care
Community commissioned care
CQC-registered, clinically led care packages for commissioners — designed for safe community delivery, clear outcomes, and dependable mobilisation when timelines are tight.
Working in partnership with commissioners
We collaborate with ICBs, NHS Trusts, Local Authorities, case managers, and discharge teams to deliver structured, person-centred packages in people’s homes and community settings.
Our model is built for complex and high-dependency needs as well as longer-term commissioned pathways. Clinical oversight, risk assessment, care planning, regular review, and clear escalation sit at the centre of every package — so safety, continuity, and outcomes remain visible throughout the contract lifecycle.
We understand discharge pressure, funding approvals, and locality priorities. Where assessment and location allow, we mobilise urgently and keep partners informed with practical updates rather than process noise.
Who we partner with
Community commissioned care at Zentar is designed for organisations that need safe, reportable delivery — not one-off cover alone.
- ICB and Continuing Healthcare (CHC) teams
- NHS Trusts and hospital discharge teams
- Local Authorities and social care commissioning
- Case management companies
- Neuro-rehabilitation and step-down pathways
- Personal health budgets and jointly funded packages
What commissioned, regulated care means with Zentar
Our community services are CQC-registered and delivered under clear clinical and operational governance. For commissioners and families, that means care that is planned, monitored, and accountable — with staff trained for the needs they support and escalation routes that work in practice.
- Person-centred care plans reviewed against changing clinical and social need
- Clinical Lead and Service Delivery Manager oversight on packages
- Safeguarding, infection prevention, and duty-of-candour aligned practice
- Transparent documentation, KPI tracking, and collaborative review meetings
- Workforce matching for skill, continuity, and sustainability of the package
Clinical and community capability
Packages can range from structured daily visits to higher-intensity or continuous support, depending on assessment, risk, and funding arrangements. We regularly support children, young people and adults with:
-
Complex care at home
Long-term and high-dependency support with clinical leadership, structured routines, and clear risk management in the home environment.
-
Neurological pathways
Support for acquired or traumatic brain injury, stroke rehabilitation needs, Parkinson’s, and related neurological conditions.
-
Clinical interventions
Packages that may include tracheostomy or ventilator support, enteral feeding, catheter and bowel care, and post-ICU step-down — subject to competency and assessment.
-
Learning disability & autism
Person-centred support that protects dignity, promotes independence, and aligns with behavioural and communication needs.
-
Rehabilitation & recovery
Goal-led support that helps people regain confidence, mobility, and daily living skills after hospital or specialist rehab.
-
Dementia & long-term conditions
Familiar routines, safety-focused practice, and compassionate support that keeps people as independent as possible at home.
-
Respite care
Planned or short-notice respite that gives families and carers a break while maintaining safe, familiar, person-centred support.
-
Community access & activities
Support to access the community, with activities led by our activities coordinator — building confidence, routine and meaningful days out.
-
In-school & SEND support
Where young people are under our care, support can continue in school — including SEND support — working alongside education staff for the best outcomes.
-
Palliative care
Compassionate end-of-life and palliative support at home, focused on comfort, dignity, symptom management and close partnership with families and clinical teams.
-
Live-in carer services
Continuous live-in care in the person’s own home — providing day-and-night presence, routine support and continuity for complex or high-dependency packages.
Respite, community life and school support
Commissioned packages are not only about clinical need at home. We also help people stay connected to everyday life — and make sure support travels with young people into education where that is part of their plan.
-
Respite that holds the package steady
Respite care for children, young people and adults — giving families and unpaid carers time to rest while the person continues to receive consistent, regulated support. Packages can be planned in advance or arranged where capacity allows for short-notice need.
-
Community access with our activities coordinator
Support to access the community, including leisure, social and developmental activity. Sessions can be shaped with our activities coordinator so people build skills, confidence and enjoyable routines beyond the home.
-
Care that continues in school
For young people under our care, support can continue within school settings, including SEND support where needed. We work alongside schools and education teams so care stays joined-up across home and classroom — protecting continuity and aiming for the best outcomes for the young person.
Supporting CHC and commissioned frameworks
We are experienced in working within Continuing Healthcare, joint-funded arrangements, personal health budgets, and locality contracts. Our focus is measurable outcomes, funding compliance, and stable staffing — so packages remain safe and sustainable over time.
- Clear care documentation and commissioner-ready reporting
- Collaborative MDT and review meeting participation
- Workforce plans aligned to locality capacity and market-shaping priorities
- Escalation pathways that keep risk visible early
Hospital discharge and rapid mobilisation
When someone is ready to leave hospital or a rehab unit, delays in community support put pressure on beds and increase readmission risk. We work with discharge and commissioning teams on pre-discharge assessment, transition planning, and short-term stabilisation where needed.
Where assessment and location allow, we prioritise urgent starts and keep communication practical: who is attending, what the care plan covers, and how outcomes will be reviewed. Temporary packages can also transition into longer-term commissioned care when needs continue.
How to refer a package
We welcome referrals and partnership conversations. Share the essentials below and our commissioning team will respond promptly to arrange assessment and mobilisation planning.
-
Referral to our team for review
Send the referral through so our commissioning team can review eligibility, fit and next steps.
-
Care plan and risk assessment
Existing care plan assessments, risk assessments, or known safeguarding considerations.
-
Mobilisation plan
Agree how the package will be rolled out — staffing, start date, handover and review milestones.
-

Urgent referrals are our speciality. Our dedicated team assesses and mobilises packages quickly and safely right across the UK — and has rolled out full care in as little as 24 hours.
