Zentar Health & Social Care

Supported living

Zentar operates supported living homes across London with staff on site 24/7 — helping service users step down from higher levels of care and build independence or semi-independent living, with quality support that feels like home.

What supported living is

Supported living is a home of your own — with care and support wrapped around it — so people can live as independently as possible. It is not a care home or an institution. Service users keep privacy, choice and control over day-to-day life, while staff help with the practical and personal support needed to stay safe, well and connected.

For many people referred by Local Authorities, ICBs and NHS partners, it is a step-down route from hospital, residential or crisis settings into a more ordinary life: cooking, routines, relationships, local services and personal goals — with 24/7 staff available when they are needed.

Our model — 24/7 staffing across London

Our supported living facilities across London are staffed around the clock. That continuous presence gives service users and commissioners confidence that help is available day and night — for personal care, medication prompts, emotional support, risk management and everyday living skills.

Working with service users and referring organisations

Quality supported living is a partnership. We work closely with each service user and with the Local Authority, ICB, NHS team or case manager who refers them — so goals stay aligned, progress is visible, and support adapts as confidence grows.

Activities and everyday life

Support is not only about keeping people safe — it is about helping them build a meaningful day. Our teams encourage routines, hobbies, education, volunteering, fitness and social connection that match each person’s interests and goals.

Activities may happen in the home or out in the local community. We support people to use local shops, leisure, health services and transport where appropriate — so skills learned in the house transfer into ordinary life.

What quality provision looks like with Zentar

We design services around choice, dignity, privacy and human rights — so people can lead confident, inclusive lives in the community. That means the right support, in the right home, with a culture that promotes independence rather than institutional routines.

Local authorities, ICBs and partners — refer a package or set up a new provision

If you have a service user ready for supported living now, make a new referral through our care referral form — our team will review eligibility, fit and next steps, and respond promptly to arrange assessment and mobilisation planning.

If you are a Local Authority, ICB, NHS organisation or referring partner and would like to discuss working with Zentar Health & Social Care to set up a new supported living provision for your service users, please get in touch. We can explore locations, staffing models, step-down pathways and how a new home could support independence in your area.
